    4. Financial Situation: If relevant, provide information about the individual’s financial situation. Include any sources of income, benefits, or support they may be receiving:

    5. Affirmation of Truth: Include a statement affirming that the information provided is true to the best of your knowledge. For example: “6. I affirm that the information provided in this affidavit is true and correct to the best of my knowledge.”
    6. Signature and Date: Sign and date the affidavit. Depending on local laws, you may also need to include a statement about how you are signing under oath.
