– Customers can also send an SMS to 12580 and include their fax number or e-mail address where the application form can be forwarded to.
Completed forms should be e-mailed to 12580@vodacom.co.za. Customer can also complete and submit their forms at Vodacom participating stores. A full list of stores can be viewed on http://www.vodacom.co.za/
